Position Summary:
Reporting to the Day Program Manager and based on the policies, procedures and standards of the Alzheimer Society of Chatham-Kent, the Day Program Coordinator will ensure the quality of client care through the implementation of client-centred care supported by stimulating activity programming. This position will also include the utilization of community resources.

This position reports to the Manager, Day Programs and may have up to 10 employees reporting to them. 
Key Responsibilities:

  • Supervise the coordination of activities for the Day Program
  • Set a risk-free Day Program environment
  • Participate in and ensure implementation of stimulating activity programming for clients
  • Participate in the on-going evaluation of programming to meet each individual client’s needs and interests
  • Assist clients with personal care as needed (toileting, special dietary needs/feeding, grooming), as well as designated tasks (ostomy/colostomy care, catheter support, injection, oxygen, etc.) when appropriate as per unregulated care providers guidelines
  • Assistance with individual Physical Therapy Program, when appropriate as per unregulated care providers guidelines
  • Provide medication support as needed (medications, creams, injections, inhalers), when appropriate as per unregulated care providers guidelines
  • Assist clients with mobility as needed to reduce fall risk
  • Provide support to the other program employees, volunteers and students in implementing the program
  • Report any serious occurrence to the Day Program Manager
  • Attend and participate in staff meetings, as well as interdisciplinary meetings as needed
  • Complete a monthly calendar of events that can be given to clients and their caregivers
  • Maintain client transportation, beverage, and birthday lists
  • Arrange any day-to-day transportation or meal planning
  • Effective daily communication with caregivers about each client’s care needs or observations in the Day Program
  • Communication with Day Program Manager when need to consult on client’s care plan
  • Consult with the Day Program Manager on the ongoing eligibility of clients for the program
  • Purchase activity supplies for the program
  • Ensure emergency binder is kept up to date
  • Complete daily progress notes, etc. in a timely and accurate manner in AlayaCare
  • Participate in Joint Health & Safety Committee meetings and take/maintain training for being JHSC certified.
  • Performs other duties as assigned.

  • Ability to understand that those with dementia are all unique individuals who require specialized attention to their personhood and that care should be delivered accordingly
  • Knowledge and work experience with people affected by Alzheimer’s disease or a related dementia is required
  • Sensitivity to the needs of those affected by dementia
  • Ability to utilize best practice care within scope of PSW role
  • Demonstrated professional work ethic and behaviours with internal and external contacts
  • Demonstrated effectiveness working with minimal supervision and proven reliability and trustworthiness.
  • Assesses risk and applies good judgment in any situation
  • Demonstrated excellent written and verbal communication skills.
  • Proven ability to handle confidential information with discretion, be adaptable to various competing demands, and demonstrate the highest level of customer/client service and response.
  • Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness.
  • Professionalism to be always maintained.
  • Represents the Alzheimer Society professionally in the community.

  • Takes an active role in promoting and protecting personal health and safety and the health and safety of others, both staff and consumers. [Sec. 28(2) OHSA] 
  • The primary work location is the Main Office at 36 Memory Lane, Chatham, Ontario or 3-212 Elizabeth Street, Wallaceburg, Ontario
  • Demonstrated experience working with individuals in a community setting.
  • Demonstrated experience working with persons living with dementia is required along with an in-depth knowledge of Alzheimer’s disease and related dementias including behavioural strategies and GPA techniques. 
  • Must have excellent written and oral communication skills; the ability to communicate fluently in French or other language(s) is an asset.
  • Must have the ability to work independently in a professional, self-directed, and collaborative manner.
  • Experience working in interprofessional teams is an asset.
  • Must have proficiency with Microsoft Office Suite applications. 
  • A valid driver’s license and access to a vehicle are required.
  • A solid understanding of personhood and person-centered care
  • Ability to utilize best practice care within scope of PSW role.
  • Demonstrated professional work ethic and behaviours with internal and external contacts.
  • Demonstrated effectiveness working with minimal supervision and proven reliability and trustworthiness.
  • Excellent written and verbal skills for both receiving and providing information.
  • Ability to work in a multi-disciplinary environment
  • Flexibility in work schedule (occasional evening and weekend hours required)
  • Flexibility to work from home
  • Ability to work independently and as a member of a team
  • Valid driver’s license
  • Access to a dependable vehicle

Academic/Educational Requirements:
  • A certificate in Personal Support Worker, along with current First Aid, and Cardiopulmonary Resuscitation Certificate.
  • Post Secondary Degree or diploma in Social Sciences, experience in the field of Gerontology or Activation (or equivalent)
  • Current First Aid/CPR Certification
  • Training in Gentle Persuasive Approaches (GPA), Montessori or U-First would be an asset

Physical Demands: 
  • At times, light lifting is necessary (materials/display boards for information fairs or other community events).
  • Standing for long periods.
  • Driving.

Hours of Work:
  • Guaranteed 32 hours per week.
  • Flexibility in work hours is required.

Hiring Zone:
$22.00 per hour to $27.00 per hour (plus $3.00 wage enhancement for each hour working directly with clients).
